Infection After Tooth Extraction: Normal Healing vs. Warning Signs

Most tooth extractions heal without any trouble. But infections after extraction, including wisdom tooth removal, do happen, and a small number become serious. Knowing what normal healing looks like, and which signs mean you should act quickly, can make a real difference.

What normal healing looks like

In the first two to three days after an extraction it is common to have some soreness, mild swelling of the cheek, a little oozing of blood, and tenderness when you open your mouth. These symptoms should gradually improve each day, not get worse.

Warning signs to call the office about

Call us the same day if you notice any of the following:

  • Pain that gets worse after day 3 or 4 instead of better
  • Swelling that increases after the first few days
  • A bad taste or bad odor in your mouth, or pus at the extraction site
  • A fever
  • Difficulty opening your mouth that is getting worse

Severe pain a few days after extraction can also be a dry socket, which is not an infection but still needs to be treated. Either way, please call rather than waiting.

When to go to the emergency room

Go to the nearest emergency room or call 911 if swelling spreads into your cheek, jaw or neck, or if you have trouble swallowing or breathing. Infections in the face and neck can spread quickly and can become life-threatening. Do not wait for an appointment.

How we help prevent infection

Prevention starts before the extraction. We review your medical history and medications, evaluate the tooth and surrounding bone, use sterile technique, and give you written aftercare instructions. When an antibiotic is appropriate for your situation, we prescribe it. We do not use antibiotics routinely for every patient.

What you can do at home

  • Follow your aftercare instructions closely, including any rinsing instructions
  • Do not smoke or vape while you heal, as it slows healing and raises the risk of problems
  • Do not poke at the area or spit forcefully
  • Take any prescribed medication exactly as directed
  • Keep your follow-up appointment
